8. The completion of two infrastructure projects, both titled “Installation of Solar
Street Lighting for Various Barangay,” with contract amounts of ₱2,712,000.00 and
₱2,929,287.00, respectively, was reported at 100% completion in CY 2022. However,
upon inspection by a COA Technical Audit Specialist, it was determined that the
actual physical accomplishments were only 75.61% and 77.06%, respectively,
resulting in a total overpayment of ₱1,333,500.00.
8.1. On June 29, 2022, the Municipality entered into a contract for the installation of
solar street lights in Barangays Bong-ao, East Balabag, and Balugo, with a contract
amount of ₱2,712,000.00.
8.2. The project was reported to be 100% complete as of August 11, 2022, based on the
Statement of Work Accomplished (SWA) attached to the payment of final billing
covered by Check No. 86268455 and Disbursement Voucher (DV) No. 100-2023-
09-3929 dated September 12, 2023.
8.3. Additionally, the Certificate of Inspection/Completion and Acceptance dated
August 11, 2022, signed by the General Foreman, the former Municipal Engineer,
and Municipal Mayor, certified that the project was satisfactorily completed in
accordance with the approved plans and specifications.
8.4. Similarly, on July 27, 2022, the Municipality entered into another contract amount
of ₱2,929,287.00, for the installation of solar street lights in Barangays North and
South Poblacion.
8.5. This second project was also reported as 100% complete as of October 14, 2022,
as reflected in the Statement of Work Accomplished (SWA) submitted with the
final billing, which was paid through Check No. 88475944 and Disbursement
Voucher (DV) No. 100-2023-11-5118 dated November 28, 2023.
8.6. The Certificate of Inspection, likewise dated October 14, 2022, and signed by the
General Foreman and the former Municipal Engineer, stated that the project was
completed 100% in accordance with the plans and specifications of the contract.
8.7. However, an ocular inspection conducted by the COA Technical Audit Specialist
(TAS) on September 10, 2024, revealed significant discrepancies. According to the
Inspection Reports dated October 7, 2024, the actual accomplishment for the two
projects was only 75.61 percent and 77.06 percent, respectively, which is far below
the reported 100 percent completion.
